Medication Therapy Management (MTM) is patient-specific medical care provided by pharmacists to optimize drug therapy and improve therapeutic outcomes. MTM includes a broad range of clinical pharmacy activities, including but not limited to the following actions.

Introducing a new medication or converting from a brand to a generic drug may or may not translate into immediate cost savings unless a proactive conversion program is established. Anuco will work with our client’s staff to establish inventory par levels and return programs.

Polypharmacy refers to a patient concurrently taking multiple medications to manage co-existing health problems. Multimorbidity, commonly defined as the co-existence of two or more chronic health conditions, is common in the older population. Multiple chronic conditions increase the complexity of therapeutic management for both health professionals and patients and negatively impact health outcomes.


Anuco will identify polypharmacy through our retrospective MUE process and monthly reports.  Our clinical pharmacists can identify high-risk drug combinations, patients at risk for inappropriate polypharmacy and alert practitioners by reviewing these reports.

Prior authorizations are not just a way for cost control. Anuco’s pharmacist directs your providers to prescribe formulary items when at all possible. When we receive orders for non-formulary medication, the Prior Authorization Team reviews the patient’s medical record, current clinical presentation, and treatment provided to date and makes appropriate recommendations by comparing gold, national, and community standards. 

This process is needed to minimize prescription costs bypassing unnecessary or expensive treatment options and ensuring that what is being prescribed is medically necessary and appropriate for the patient

Medication Reconciliation is a review of medications patients were prescribed at the hospital and comparing them against what they were taking before admission.

Medication-use evaluation (MUE) or medication evaluation is a performance improvement method that focuses on evaluating and improving medication use processes to achieve optimal patient outcomes. MUEs are performed to attain process-oriented key objectives:

We provide three types of medication evaluation: Prospective MUEConcurrent MUE, and Retrospective MUE.

The multiplicity of medications available, the complexities surrounding their safe and effective use, and differences in their relative value make it necessary to have medication-use policies that promote rational, evidence-based, clinically appropriate, safe and cost-effective medication therapy.

Our pharmacists work through this ongoing process to assist with establishing policies on the use of drugs, therapies, and drug-related products and identify those that are most medically appropriate and cost-effective best to serve the health interests of a given patient population.

Anuco’s experienced clinical services team supports all aspects of the Pharmacy and Therapeutic (P&T) Committee process. We work with clinical teams to support P&T Committee meetings, assist with P&T agendas and supporting information packets, and provide expert clinical pharmacist advice.

In addition, Anuco’s clinical specialists will be available to assist with a formalized Disease Management Evaluation (DME) program.  P&T Committees often use DMEs to evaluate the effectiveness of current & proposed drugs and identify drug-related problems, assess drug control systems, criteria, and protocols, and identify areas where further education of providers and patients is needed.

Anuco will provide pharmacy-related training for staff, when applicable, through a variety of programs, including staff orientation, in-service sessions, MUE educational interventions, and drug monographs.  In addition, upon request, Anuco’s licensed pharmacist will perform annual (at a minimum) in-service training on pharmacy-related material according to a schedule

Anuco has designed and maintained a set of Pharmacy Quality Metrics (PQM) to identify medication-related incidents to improve the efficiency and effectiveness of the pharmacy program.  The PQM meeting aims to identify internal and external processes that can be improved to provide safe and exemplary service to our clients.  Once an issue has been identified, we work with our client on a resolution.  Our pharmacists collect potential incidents at the site-level Quality Management Meetings and analyze the data through the medication error analysis tool, which are then discussed at all PQM meetings.

National guidelines rarely address the specific needs of patients confined in correctional
institutions. When they address this population, they frequently do not review the special
populations or the unique operational issues that make medical therapy management challenging
in the correctional arena.  
Anuco’s utilization management team will work in tandem with the medical team to fashion
guidelines or algorithms that match the patient population being treated.  Such guidelines are
critical evidence-based and patient-focused outcome-driven tools essential to good pharmacy
management.

Anuco’s medication room inspections use a multi-level approach that ensures patient safety,
employee accountability, institutional security, and regulatory. The goal is to establish, maintain,
and exceed standards between accreditations, not just to prepare for them. 

We conduct monthly on-site pharmacy inspections at each medication storage area as per state
and federal regulations and the accreditation standards of the National Commission on
Correctional Health Care (NCCHC), the American Correctional Association (ACA), and the
Joint Commission (JC). Results of these inspections will be recorded and reported to the medical
staff. Follow-up on corrective actions required will be monitored to ensure any deficiencies are
corrected on time. The inspection is designed to ensure not only that all medication areas are
following state and national regulations but also to: 

  • Identify expiration dates, recall medications, and secure storage under the proper condition of sanitation, temperature, light, moisture, ventilation, and segregation. 
  • Review inventory practices (i.e., PAR levels) and accountability 
  • Provide in-service education and training to clinic staff, as applicable. 
  • Assume responsibility for the destruction or removal of unwanted dangerous drugs and any controlled substances as prescribed by law and regulations.  
  • Maintain a log of all visits and activities in the facility indicating dates and other pertinent data; such records are available for inspection by state drug inspectors upon request.  
  • Make routine inspections of drug storage areas, patient health records, and review the drug regimen of each patient at least once a month—report irregularities, contraindication, drug interactions, etc., to the medical staff.  
  • Meet all other responsibilities of a consultant pharmacist as outlined in the Board regulations and federal or state laws and consistent with quality patient care.  

A licensed Anuco consulting pharmacist will make the monthly on-site inspections and be
available to staff to discuss other pharmacy-related topics, including inspection compliance, drug
regimen reviews, and formulary management. This multi-level approach provides a valuable tool
for the facility to ensure patient safety, employee accountability, institutional security, and
